When we had our presentations by a college office worker, everyone received one of these forms except for me. Does anyone know where I can find it on the school's website? It's due in May, I believe.

Actually, the thing due May is a form on Naviance. Whatever you didn't receive could be found in the college office.

On naviance, go to the "About Me" Tab. On the left side, click "Student questionnaire for counselor recommendation."
