Where do I go to hand in the health form?

I have no clue where to go to hand in the health room. Can anyone let me know?

I think you just give it to your teacher (my health teacher requested it). I got my health contract via email and I just printed out and sign it.

you mean health forms for PSAL teams? You give them to a coach if you're planning to tryout for a team, or if your a returning member for the team

It was the health forms. Thanks for the answers.
1 more question though. I bought a lock for the gym lockers from the school, but it seems that the lock doesn't work (The dial stops after entering the combination, but the lock will not come out). Should I go to the gym on monday and have it exchanged for a new one? (Not sure what period I should go, I don't have gym on monday's)

You mean after you put in your combination, the lock wont open?
after entering your combination your supposed to turn the dial either left or right, I forget, and it should stop moving, like it feels jammed and then pull down while it's jammed and it should open.
It was in the directions that came with the lock. these master locks aren't like the ones you would get from staples.

Erm, you mean after entering the combo, you turn the dial to the left and hold it there and pull the handle open? :P

Yeah. I'm 100% sure I did it correctly, but I can't pull the handle open. Did it like 20 times last night.

Trying banging it hard on a hard surface.
Mine got stuck and that seemed to do the trick.

They SHOULD be able to exchange it, seeing it as they sold you a faulty product.
